Job Description
Job Title: Case Planner
Position Objective:
To provide social services to families including case planning, advocacy and crisis intervention. Avoid foster care placement and or re-entry in the system.
Essential Functions:
Conduct intakes.
Assess risks and develop case plans along with family participation.
Assessment of clients, their families and existing informal and formal supports.
Conduct home visits and office visits.
Conduct field visits to any other locations relevant to assist families ( schools, mental health ).
Assist families maintain medical, financial, educational and social resources.
Facilitate groups.
Facilitate referrals to other providers and monitor utilization of services.
Request and secure supportive documentation in a timely manner.
Conducts on going assessment on service plan goals and integrates any new developments in the course of services.
Facilitate Family Team Conferences for all families when recahing its six month of services.
Complete recordings and FASPS (service plans) within the time frames set by agency and funding source.
Knowledge of ACS policies and procedures
Position Requirements:
Education: BSW Preferred
Previous child care welfare experience preferred not required
Bilingual Spanish a plus.
Computer proficient. Good communication, verbal and written skills.
