CLASSIFICATION
Staff track position in health/social service arena. This position is ideal for a community minded individual who is seeking to link needed resources to older adults, adults with disabilities, and family caregivers; especially within the ethnic minority populations in the County of Los Angeles. The position is an excellent opportunity for building a career in social services and/or preparation for graduate studies (i.e. Social Work, Psychology, etc.)SUMMARY OF DUTIES
Under the direction of the Program Director, s/he will be responsible for:
Evaluating clients eligibility for service, conducting in-home assessments and coordinating service arrangements/authorizations;
Developing individualized care plans that will ensure a seamless system of services that is culturally sensitive and language appropriate;
Exploring appropriate options for short-/long-term care and supportive resources with client and family geared at maintaining client independence, autonomy and self-determination;
Developing and maintaining a network of support for clients (e.g. through family, community groups) through case monitoring;
Developing and maintaining community relationships with public and private service organizations, both ethnic and mainstream;
Availability to work some evening and weekend hours as necessary.
Work with a multidisciplinary mental health treatment team
Participate in monthly Care Plan Team Conference and other group supervision
Work with multidisciplinary team in providing 24/7 crisis intervention
Responsible for communicating effectively (verbal & written) with clients, family members, service providers, co-workers, Adult Protective Services, other public and private departments, and service providers.
Responsible for all monthly paperwork, data gathering and project billing specific to own client caseload.
Participate in production of project outreach and publicity materials.
Advocate for issues relating to and affecting older adults, adults with disabilities and family caregivers of ethnic minority populations.
Other responsibilities as assigned by the Program Director as needed.
