Contemporary Guidance Services, Inc. a not- for-profit organization serving individuals with disabilities is seeking an experienced, dedicated professional for the position of Day Habilitation Assistant. Since 2004, CGS has provided Group Day Habilitation without Walls Programs for individuals with developmental disabilities in Manhattan, Bronx and Brooklyn. Throughout the school year, a CGS Community Outreach professional takes part in school transition fairs and parents’ workshops, meets with graduating students and their families to identify what services and programs they want, and discusses feasible choices for transition from school to life as adults in their communities. The program centers on a variety of work experience opportunities in the community - hospitals, nursing homes, stores, entertainment complexes are just a few! - and on discovering and enjoying social, recreational and physical fitness activities and community resources.
The Day Habilitation Assistant promotes the concepts of choice, individualized services and supports and consumer satisfaction by providing intensive, individualized program planning to the program consumers. Duties include but not limited to:
•Actively involve the consumer in the Day Rehabilitation Program’s volunteer, recreational and educational activities.
•Assist the consumer to explore and develop experiences that provide them with a good quality of life by planning and implementing a monthly activity schedule.
•Work closely with the Case Manager to match the consumer with prospective volunteer sites through person-centered planning; this will be based on the consumer’s individual preferences.
•Provide intensive individualized services and job coaching at the volunteer sites and advocate on the consumer’s behalf.
•Provide family support through training and crisis intervention when appropriate.
•Follow the 3 IP philosophy and the 5 Governing Principles of OMRDD.
•Provide daily bus supervision
•Submit weekly case notes that are complete, accurate, updated and neat.
•Maintain records and statistics in accordance with the program and funding agency requirements and
•Complete all reports and documents in a timely manner; these include bus arrival/departure forms, daily case notes, and staff and consumer attendance records.
The successful candidate has a BA in psychology or related field or AA degree and 1 – 2 years’ experience in working with individuals with developmental disabilities. Must be willing to travel throughout areas in Brooklyn.
