Job Functions:
1. Assist patients and their families in the adjustment to kidney disease and dialysis treatment.
2. Assist patients in accessing community resources and eligible benefits.
3. Conduct psychosocial assessments and routine social work documentation.
4. Assist patients in the kidney transplant evaluation process and work directly with transplant centers.
5. Participate in the Interdisciplinary Plan of Care and coordinate care with a team of nurses, physicians, dietitians, and administrators.
6. Facilitate education and support groups
