Specific Education, Certification and Skill Requirements (if applicable): Requirements: B.S. or B.A. in social work, social health sciences or nursing with two or more years experience in case management and/or in child, family or community services; Must speak, read and write English and Spanish and possess a valid driver’s license and an insured vehicle.
Job Description: Responsible for providing care coordination and health education for low risk, at risk, and socially high-risk pregnant patients, infants and their families.
