Reporting to the Director of Social Services, the Caseworker is responsible for working in the Homelessness Intervention Program (HIP) providing aftercare support services to formerly homeless families, crisis intervention, needs assessment, referrals, liaison and advocacy.- Conduct intake interviews and needs assessments for families
- Provide crisis intervention
- Conduct individual and family counseling sessions
- Complete service plan with family monthly
- Conducts monthly home visits
- Conduct apartment inspections
- Conduct client meetings
- Coordinate and follow up on referrals for concrete services
- Complete discharge planning for families
- Maintain case records for families
- Serve as liaison with landlords, realtors, and governmental agencies
- Perform related duties as assigned
