Job Description: The West Side Federaton for Senior and Supportive Housing,Inc. (WSFSSH),is a NFP organization that provides housing with social services to low income elderly, to people with chronic mental illness and to people who have experienced homelessness. Currently, WSFSSH has an opening for a Case Manager in Valley Lodge, a transitional shelter. Valley Lodge accepts men and women over the age of 50 who have mental illness and are in the NYC shelter system. The Case Manager carries a small caseload of residents and works with each resident to obtain and maintain medical care, obtain and maintain psychiatric care, obtain entitlements and to move to permanent housing. Case manager also runs 1-2 groups weekly and participates in resident meetings.
